Alpha lipoic acid (ALA) is a naturally occurring compound with antioxidant properties, and its use in cardiovascular disease (CVD) is primarily supported by early scientific research rather than longstanding traditional use. Several mechanisms have been proposed for ALA’s potential cardiovascular benefits, including its ability to reduce oxidative stress, improve endothelial function, and decrease inflammation—all of which are implicated in the pathogenesis of CVD. Some clinical studies have shown that ALA supplementation can improve markers of endothelial function, such as flow-mediated dilation, and may reduce levels of inflammatory markers like C-reactive protein (CRP). Additionally, ALA can improve insulin sensitivity, which could indirectly benefit cardiovascular health in patients with metabolic syndrome or diabetes.
However, the overall clinical evidence remains limited and somewhat inconsistent. Most studies are small, short in duration, and often focus on surrogate endpoints rather than hard cardiovascular outcomes (such as myocardial infarction or mortality). A 2018 systematic review noted some positive effects on blood pressure and lipid profiles, but concluded that more robust, long-term randomized controlled trials are needed. Thus, while there is some scientific rationale and preliminary evidence supporting the use of alpha lipoic acid for cardiovascular health, the strength of the evidence is modest (rated 2/5), and ALA is not yet a mainstream or guideline-recommended therapy for CVD.
