Cynanchum species have a long history of use in Traditional Chinese Medicine (TCM) and other traditional medicine systems. In particular, species such as Cynanchum atratum (known as Bai Wei) have been recorded in classical texts as remedies for skin infections, abscesses, sores, and carbuncles. Traditional herbalists have utilized Cynanchum for its purported abilities to "clear heat," "detoxify," and reduce swelling, which aligns with the TCM conceptualization of carbuncles as manifestations of internal heat and toxicity. Preparations of roots or whole plant are often included in multi-herb decoctions to address such conditions.
However, scientific studies directly evaluating the efficacy of Cynanchum species against carbuncles are limited. The evidence for its use is therefore primarily traditional, based on centuries of empirical use and inclusion in authoritative herbal pharmacopeias. Some modern phytochemical studies have identified anti-inflammatory and antibacterial compounds in certain Cynanchum species, but these findings are preliminary and have not been specifically correlated with clinical outcomes for carbuncles in controlled trials. As such, while there is a plausible pharmacological basis, the support for its use in this context remains largely traditional, with limited scientific validation.
