Dextran is a complex branched polysaccharide that has been widely used in medicine, particularly as a plasma volume expander in intravenous solutions. Its primary scientific justification for use relates to its effects on the capillary and vascular system. Dextran solutions (notably Dextran 40 and Dextran 70) are used to increase blood volume in cases of hypovolemia and shock. By increasing plasma volume, dextran helps maintain capillary perfusion and improves microcirculatory flow, thus supporting capillary function. Additionally, dextran has antithrombotic properties, as it reduces blood viscosity and inhibits platelet aggregation, further contributing to the maintenance of capillary patency and reducing the risk of microvascular thrombosis.

Numerous clinical studies and reviews (e.g., British Journal of Anaesthesia, 2009; Cochrane Reviews, 2010) have documented dextran’s ability to prevent capillary sludging and improve microcirculatory flow, especially in surgical and trauma patients. However, its use is tempered by potential side effects such as anaphylactic reactions, coagulopathy, and kidney injury. Dextran is not used as a nutritional supplement, but as a pharmacological agent administered under medical supervision. Its role in supporting the capillary system is thus well-supported by scientific evidence, but clinical use is carefully monitored due to associated risks.
