Ornithine alpha-ketoglutarate (OKG) is promoted in the bodybuilding community mainly for its supposed ability to stimulate muscle growth, improve recovery, and enhance performance by modulating amino acid metabolism and increasing growth hormone secretion. Some scientific rationale exists: OKG is a salt combining the amino acid ornithine and alpha-ketoglutarate, a key molecule in the Krebs cycle. Ornithine itself is involved in the urea cycle and may reduce ammonia accumulation during intense exercise, theoretically improving endurance and recovery. OKG has been investigated in clinical settings (primarily in trauma, burn, or elderly patients) for its potential to support protein synthesis, wound healing, and nitrogen retention.

However, direct and robust scientific evidence supporting OKG supplementation for bodybuilding—specifically in healthy, resistance-trained individuals—is limited. Most studies are small, short-term, or conducted in populations with increased catabolic stress rather than athletes. While some findings suggest improved nitrogen retention and reduced muscle breakdown in clinical contexts, translation to muscle hypertrophy or strength gains in athletes is unproven. A few studies suggest possible improvements in muscle mass in cachectic patients, but effects in healthy adults are not well established. Systematic reviews and sports nutrition guidelines do not currently endorse OKG as a proven ergogenic aid for bodybuilding. Thus, while a plausible biological mechanism exists, the actual strength of evidence in the context of bodybuilding is weak.
