Bael (Aegle marmelos) is a fruit-bearing tree commonly used in traditional Ayurvedic and Unani medicine, especially in India and surrounding regions. Its use for treating "blood in stool" (hematochezia) primarily arises from traditional practices rather than robust scientific validation.

In Ayurveda, bael fruit pulp (often referred to as "bilva") is considered to have astringent, anti-diarrheal, and anti-inflammatory properties. Traditional healers have prescribed bael for a range of gastrointestinal disorders, including dysentery, chronic diarrhea, and sometimes for symptoms associated with ulcerative colitis, which can include blood in the stool. The rationale is that bael may help soothe the intestinal mucosa, reduce inflammation, and firm up stools, thereby potentially reducing minor bleeding associated with irritated or inflamed bowel tissue.

However, scientific studies directly evaluating bael's efficacy for blood in stool are sparse. Some preclinical studies have shown anti-inflammatory and mucosal protective effects of bael extracts in animal models of colitis and diarrhea, but controlled clinical trials in humans for this specific indication are lacking. Thus, the use of bael for blood in stool is mainly justified by a long-standing tradition and anecdotal reports rather than strong scientific evidence.
