Earthworm (commonly referred to as "Di Long" in Traditional Chinese Medicine, TCM) has been historically used in various Asian medical systems to support the treatment and prevention of blood clots, as well as to improve blood circulation. This traditional use is based on the belief that earthworm possesses properties that "clear heat" and "unblock channels," facilitating smoother movement of blood and reducing stasis, which can contribute to clot formation. The key active component often cited is lumbrokinase, an enzyme complex found in earthworms, which is believed to have fibrinolytic (clot-dissolving) activity.

While there is some preliminary scientific research in vitro and in animal models indicating that lumbrokinase and related compounds can break down fibrin (the protein meshwork forming clots), high-quality clinical trials in humans are limited and evidence is not robust. Most of the support for earthworm's use in clot prevention or treatment comes from traditional practices and case reports rather than systematic research. Therefore, its use is best described as being justified by tradition, with low to moderate supporting evidence from modern studies. Professional guidelines do not include earthworm as a standard therapy for blood clot prevention or treatment, and safety profiles for long-term use remain inadequately defined.
