Phellodendron bark, also known as Huang Bai in Traditional Chinese Medicine (TCM), has a long history of use for urinary tract and bladder issues. In TCM, it is considered a "bitter and cold" herb used for "clearing heat" and "dampness," and is often included in formulas to address symptoms that could correspond to modern diagnoses of bladder infections, such as painful urination, frequent urination, and urinary urgency. Historical texts, such as the Shennong Bencao Jing, document its use for urinary tract issues. The traditional rationale is based on symptom patterns rather than microbial etiology as understood today.

From a scientific perspective, Phellodendron bark contains alkaloids like berberine, which have demonstrated antibacterial properties in laboratory studies. However, there is a lack of high-quality clinical trials specifically evaluating Phellodendron bark for the treatment of bladder infections (urinary tract infections) in humans. Most evidence is based on in vitro studies or animal models, which suggest potential efficacy, but do not conclusively validate its use in clinical practice. Thus, while there is a strong traditional basis for its use, scientific validation remains limited.
