Arbutin is a naturally occurring glycoside found in plants such as bearberry (Arctostaphylos uva-ursi). It has a long traditional use in herbal medicine for urinary tract and bladder infections. The scientific rationale for its use is based on the fact that arbutin is metabolized in the body to hydroquinone, a compound with mild antiseptic properties. Several in vitro and animal studies have demonstrated that hydroquinone can inhibit the growth of common urinary tract pathogens. Some small clinical trials and observational studies suggest that arbutin-containing extracts may reduce the recurrence of urinary tract infections (UTIs), but high-quality, large-scale randomized controlled trials are lacking. Systematic reviews have concluded that while there is some evidence for efficacy, it is not robust enough to recommend arbutin as a primary treatment. Safety concerns also exist, as hydroquinone can be toxic in high doses or with prolonged use. Overall, the use of arbutin for bladder infection is partially supported by scientific evidence, but this evidence is weak to moderate at best, with most guidelines recommending standard antibiotic therapy rather than herbal alternatives.
