Henbane (Hyoscyamus niger) has a long history of use in traditional herbal medicine, primarily in Europe, the Middle East, and parts of Asia. Its main medicinal applications have revolved around its antispasmodic, analgesic, and sedative effects, which are attributed to its content of tropane alkaloids such as hyoscyamine, scopolamine, and atropine. In traditional systems, henbane preparations have sometimes been used externally (in the form of poultices or oils) to ease pain, including backache, or used internally with great caution for neuralgic and rheumatic pain. However, there is limited documentation specifically connecting henbane to the relief of backache, and much of the recorded traditional use is general for pain rather than specifically for back pain. From a scientific perspective, there is little to no modern clinical evidence supporting the use of henbane for backache. Most contemporary references note its historical use and caution against its toxicity. The use of henbane for backache is therefore justified primarily by tradition, not by scientific validation. Due to the risk of severe side effects and toxicity (such as delirium, hallucinations, and even fatal poisoning), the use of henbane is not recommended in modern practice, and any potential benefits are far outweighed by safety concerns.
