Vitamin D2 (ergocalciferol) is sometimes used to support or treat autoimmune disorders, but the evidence is moderate and primarily extrapolated from broader research on vitamin D and immune function. Most clinical and mechanistic studies have focused on vitamin D in general, with a preference for vitamin D3 (cholecalciferol), which is more effective at raising and maintaining serum 25(OH)D levels. Both forms are converted in the body to the active hormone (1,25-dihydroxyvitamin D), which modulates immune responses by influencing T cell function and cytokine production. Observational studies have linked vitamin D deficiency with increased risk and severity of several autoimmune conditions, such as multiple sclerosis, type 1 diabetes, and rheumatoid arthritis. Some small intervention trials and animal studies suggest vitamin D supplementation can reduce disease activity, but most do not distinguish between D2 and D3. Overall, vitamin D2 can help correct deficiency, which may have indirect benefits for people with autoimmune disorders, but direct evidence for its efficacy in treating these conditions is limited and weaker compared to vitamin D3. Guidelines generally recommend correcting vitamin D deficiency in autoimmune disease but do not specifically endorse D2 over D3.
