Progesterone has been explored as a potential therapy for autoimmune disorders based on its known immunomodulatory effects, particularly in pregnancy, where high levels of progesterone are associated with immune tolerance and reduced disease activity in some autoimmune conditions (e.g., multiple sclerosis and rheumatoid arthritis). Experimental and observational studies suggest that progesterone can shift the immune response from a Th1 (pro-inflammatory) to a Th2 (anti-inflammatory) profile, suppressing certain immune pathways implicated in autoimmunity. Animal models have demonstrated some benefit in reducing disease severity in conditions like multiple sclerosis. However, large-scale, controlled clinical trials in humans are lacking, and the evidence remains preliminary. Some small studies and case reports indicate possible benefit in specific contexts, but these findings are not robust enough to support routine clinical use. Current guidelines do not recommend progesterone as a standard treatment for autoimmune disorders. Thus, while there is a biological rationale and some early-stage scientific evidence, its use for this indication is not firmly established or widely validated at this time.
