Boswellia serrata, commonly known as Indian frankincense, has a long history of use in traditional Ayurvedic medicine for inflammatory conditions. In recent decades, modern scientific studies have investigated its effects on autoimmune and inflammatory disorders, particularly rheumatoid arthritis (RA) and inflammatory bowel disease (IBD). The primary bioactive compounds, boswellic acids, have demonstrated anti-inflammatory properties by inhibiting 5-lipoxygenase and modulating cytokine production. Several small-scale clinical trials and animal studies have reported beneficial effects of Boswellia serrata extracts in reducing symptoms and inflammatory markers in patients with RA and ulcerative colitis, which are both autoimmune in nature. However, these studies often have methodological limitations such as small sample sizes, short duration, and variability in extract preparations. While results are promising, larger and more rigorous clinical trials are necessary to firmly establish efficacy and safety. Currently, Boswellia serrata is often used as an adjunctive therapy for autoimmune and inflammatory conditions, with evidence supporting moderate benefit, particularly in reducing pain and swelling. Thus, its use is justified by both traditional practices and emerging scientific evidence, though the strength of contemporary evidence is moderate rather than strong.
