D-Ribose is a naturally occurring sugar that plays a key role in the synthesis of adenosine triphosphate (ATP), the primary energy carrier in cells. Its use as an athletic and exercise aid is based on its role in cellular energy metabolism. The theoretical rationale is that supplemental ribose could replenish ATP stores more rapidly after intense exercise, potentially enhancing recovery and performance.

Several small clinical studies have investigated the effects of ribose supplementation in athletes and individuals undergoing intense physical activity. Some studies have suggested minor benefits in reducing muscle soreness and improving recovery of ATP levels post-exercise, especially in individuals with compromised energy metabolism (such as those with certain heart conditions). However, research in healthy athletes has produced mixed results. Most well-controlled studies have failed to demonstrate significant improvements in exercise performance, endurance, or recovery compared to placebo.

Overall, while the biochemical rationale for ribose supplementation is sound, the clinical evidence supporting its effectiveness as an athletic or exercise aid is limited and inconsistent. Major sports and nutrition organizations do not currently recommend ribose as an effective ergogenic aid. Thus, the evidence, while scientific in nature, remains weak (rated 2/5) and further large, well-designed studies are needed to establish any clear benefit for athletic and exercise performance.
