Ligustrazin (also known as tetramethylpyrazine, TMP) is an active compound extracted from the Chinese medicinal herb Ligusticum chuanxiong. Its use in the management of arteriosclerosis is grounded both in traditional Chinese medicine and supported by some scientific research, although the quality and strength of the evidence remain limited. In laboratory and animal studies, ligustrazin has demonstrated several biological effects relevant to arteriosclerosis, such as improving microcirculation, inhibiting platelet aggregation, reducing oxidative stress, and attenuating vascular smooth muscle cell proliferation. Some small-scale clinical studies and meta-analyses conducted in China have reported benefits of ligustrazin (often in injectable form) for improving symptoms of atherosclerotic cardiovascular diseases and cerebrovascular conditions, possibly by enhancing blood flow and reducing ischemic injury. However, these studies often suffer from methodological limitations, such as small sample sizes, lack of rigorous controls, and publication bias. There is a lack of large, high-quality randomized controlled trials published in international journals. Therefore, while ligustrazin’s use has a basis in pharmacological mechanisms and preliminary clinical findings, the overall level of evidence supporting its effectiveness in treating arteriosclerosis is modest. Further well-designed clinical research is needed to confirm its clinical utility and safety.
