Danshen (Salvia miltiorrhiza) is a traditional Chinese medicinal herb historically used for various cardiovascular conditions, including arrhythmia. Modern research has explored its potential antiarrhythmic effects, with several in vitro, animal, and limited human studies suggesting possible benefits. The active compounds in Danshen, such as tanshinones and salvianolic acids, have been shown to modulate cardiac ion channels, inhibit abnormal electrical activity, and protect cardiac cells from ischemia-induced injury. Some clinical studies, mainly from China, have reported that Danshen extracts or formulations containing Danshen may reduce the frequency or severity of arrhythmias, particularly when used as an adjunct to standard therapies. However, the overall quality of evidence remains low. Most available studies are small, have methodological limitations, or are not blinded or placebo-controlled. Meta-analyses and systematic reviews generally conclude that while there is some evidence of benefit, it is insufficient for robust clinical recommendations. International guidelines do not endorse Danshen for arrhythmia management due to the need for more rigorous trials. Therefore, while Danshen has some scientific basis for its use in arrhythmia, the evidence is limited and not definitive.
