Melatonin is a hormone naturally produced by the pineal gland that regulates sleep-wake cycles. Its primary clinical use is for sleep disorders and circadian rhythm disturbances. However, research has explored its potential for supporting or treating anxiety, primarily due to its role in modulating sleep and its indirect effects on mood regulation. Several randomized controlled trials and meta-analyses have investigated melatonin’s anxiolytic (anxiety-reducing) properties, particularly in the context of preoperative anxiety and in populations with sleep disturbances who also experience anxiety symptoms. These studies suggest that melatonin may reduce preoperative anxiety and modestly improve anxiety symptoms, likely due to its sedative and sleep-promoting effects, as well as its ability to influence neurochemical systems involved in stress and anxiety (such as GABAergic and serotonergic systems). However, the evidence is not robust for generalized anxiety disorders or chronic anxiety, and most guidelines do not recommend melatonin as a first-line treatment for anxiety. Its use is generally considered adjunctive and situational rather than primary. Overall, while there is some scientific basis for melatonin’s anxiolytic effects, the evidence is limited and inconsistent, warranting a moderate-low rating (2 out of 5) for its support in treating anxiety beyond sleep-related contexts.
