Lactobacillus paracasei is a probiotic bacterium commonly used to help mitigate the side effects of antibiotics, such as diarrhea and disruption of gut microbiota. Scientific studies, including randomized controlled trials and meta-analyses, provide substantial evidence supporting the use of certain Lactobacillus strains (including L. paracasei) for this purpose. Antibiotic-associated diarrhea (AAD) occurs because antibiotics can disturb the balance of beneficial bacteria in the gut, allowing pathogenic bacteria to proliferate. By introducing L. paracasei, the gut microbiota can be partially restored, helping to outcompete harmful organisms and maintain intestinal barrier function. Several studies have specifically found that consumption of L. paracasei reduces the incidence, severity, and duration of AAD in both children and adults. For example, clinical trials have shown that L. paracasei supplementation during antibiotic therapy significantly decreases the risk of diarrhea compared to placebo. Mechanistically, L. paracasei is believed to work by producing short-chain fatty acids, reinforcing the gut mucosal barrier, modulating immune responses, and directly inhibiting pathogenic microbes. While most evidence is based on multi-strain probiotic formulations, L. paracasei is a common and well-studied component, especially in combination with other Lactobacillus or Bifidobacterium species. Overall, the scientific support for the use of L. paracasei in reducing antibiotic-related side effects is robust, though results may vary depending on the probiotic strain and the population studied.
