Lactobacillus johnsonii has been investigated for its potential to mitigate the side effects of antibiotics, particularly antibiotic-associated diarrhea (AAD) and disruption of the gut microbiota. Antibiotics can reduce populations of beneficial gut bacteria, leading to gastrointestinal symptoms and increased risk of infections such as Clostridioides difficile. Probiotics, including some strains of L. johnsonii, are studied for their ability to help restore microbial balance.

Several preclinical and clinical studies support the use of L. johnsonii for maintaining gut health during or after antibiotic treatment. For example, a randomized controlled trial (Rautava et al., 2009) found that supplementation with L. johnsonii reduced gastrointestinal symptoms and modulated intestinal microbiota in infants receiving antibiotics. Other studies, mainly in animal models, demonstrate that L. johnsonii can help maintain mucosal barrier function and reduce pathogenic overgrowth following antibiotic administration.

However, clinical evidence in adults is more limited and often involves multi-strain probiotic products rather than L. johnsonii alone. The overall body of evidence suggests a modest benefit, with some studies showing reduced incidence or duration of antibiotic-associated diarrhea and improved restoration of gut flora. As with other probiotics, efficacy may depend on the specific strain, dose, and patient population. Major guidelines generally support the use of certain probiotics for prevention of AAD, but the specific contribution of L. johnsonii is still being clarified.
