Omega-3 fatty acids, particularly eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), have been studied for their cardiovascular benefits, including their potential role in managing angina (chest pain due to reduced blood flow to the heart). Several mechanisms have been proposed for their beneficial effects, such as anti-inflammatory properties, improvement in endothelial function, reduction of triglycerides, and antithrombotic effects. However, the evidence supporting their use specifically for angina is limited and not robust.
Clinical trials and meta-analyses have shown that omega-3 supplementation may reduce the risk of major cardiovascular events, but direct evidence for reduction in angina frequency or severity is inconsistent. Some small studies have suggested a reduction in angina symptoms and improved exercise tolerance in patients with coronary artery disease, but larger trials have not consistently confirmed these findings. Major guidelines generally support omega-3 fatty acids for general cardiovascular risk reduction, particularly in patients with elevated triglycerides, but do not recommend them specifically for angina management.
Historically, the use of omega-3s for heart health has been widespread, driven by early epidemiological findings in populations with high fish consumption and low rates of heart disease. While omega-3s are safe and have overall cardiovascular benefits, their direct effect on angina symptoms is modest at best, and the evidence base (rating: 2/5) does not strongly support their use for this specific indication.
