Ligusticum, particularly Ligusticum chuanxiong (also known as Szechuan lovage or Chuanxiong), has a long history of use in traditional Chinese medicine (TCM) for promoting blood circulation and alleviating pain, including conditions described as "chest pain due to blood stasis"—which is sometimes interpreted as analogous to angina pectoris in Western medicine. The primary basis for its use in angina comes from these traditional applications, where it is often included in herbal formulas intended for cardiovascular complaints.

Scientific studies directly validating Ligusticum’s efficacy for angina in rigorous human clinical trials are limited. Some laboratory and animal studies have indicated that extracts of Ligusticum chuanxiong can have vasodilatory effects, inhibit platelet aggregation, and improve microcirculation, which theoretically could benefit angina. However, these effects have not been conclusively demonstrated in large, well-controlled human studies specifically for angina pectoris. Therefore, while there is some pharmacological rationale and preclinical support, clinical evidence remains weak.

In summary, the use of Ligusticum for angina is supported primarily by traditional medical systems, with a small but growing body of preclinical research providing some mechanistic insight. However, high-quality clinical trials are lacking, and thus the evidence is best rated as modest and traditional rather than scientifically robust.
