Vitamin D (ergocalciferol, or vitamin D2) is not traditionally used as a primary therapy for anemia, but there is some scientific evidence suggesting a relationship between vitamin D status and anemia, particularly anemia of chronic disease and iron-deficiency anemia. Several observational studies have found associations between low vitamin D levels and increased risk of anemia in various populations, including children, adults, and those with chronic kidney disease. The proposed mechanisms include vitamin D’s role in modulating inflammation and supporting erythropoiesis (red blood cell production) through effects on erythroid progenitor cells and the suppression of inflammatory cytokines that can inhibit erythropoietin production and function.
However, the evidence base is limited and mostly consists of observational studies or small randomized trials. A few interventional studies have shown modest improvements in hemoglobin levels among individuals with vitamin D deficiency who received supplementation, particularly in those with chronic kidney disease. Nevertheless, these effects are generally mild, and vitamin D supplementation is not considered a stand-alone treatment for anemia. Large, well-designed randomized controlled trials are lacking, and current clinical guidelines do not recommend vitamin D as a primary anemia therapy, except as part of general nutritional support or in the context of correcting deficiency.
In summary, while there is some scientific rationale and preliminary evidence for a role of vitamin D in supporting anemia management, the quality and quantity of evidence are limited (rated 2/5), and its use for this indication is not well established.
