Pyridoxal-5-phosphate (P-5-P) is the biologically active form of vitamin B6 and plays a critical role as a coenzyme in numerous metabolic pathways, including those involved in heme biosynthesis—the process by which hemoglobin is produced. Specifically, P-5-P is required for the function of delta-aminolevulinic acid synthase, the first and rate-limiting enzyme in the heme synthesis pathway. Deficiency in vitamin B6 can lead to impaired heme synthesis, resulting in a form of microcytic, hypochromic anemia that resembles iron deficiency anemia but does not respond to iron supplementation. This connection has been validated in both animal studies and case reports in humans, particularly in individuals with congenital disorders of vitamin B6 metabolism or with conditions causing malabsorption.
However, while there is clear scientific rationale for the use of P-5-P in vitamin B6 deficiency-induced anemia, the evidence supporting its use in broader populations with anemia (such as those with anemia of chronic disease or iron deficiency anemia) is limited. Most clinical guidelines do not recommend routine P-5-P supplementation for anemia except in documented or suspected cases of vitamin B6 deficiency. Randomized controlled trials specifically evaluating P-5-P for general anemia management are lacking, which is why the evidence is rated as 2 out of 5—reflecting a mechanistic and case-based rationale rather than robust clinical trial support.
