Podophyllum, a plant known for its resin (podophyllin), has a long history of use in traditional systems of medicine, particularly Ayurveda and Unani, primarily for its caustic, antiviral, and cytotoxic properties. In the context of anal fistula and anal fissure, Podophyllum has been used topically in traditional medicine as a caustic agent to promote healing, reduce abnormal tissue, and assist in local destruction of unhealthy tissue. However, there is no substantial scientific evidence or modern clinical trial data supporting its efficacy or safety for these specific conditions. Its main scientifically validated uses are for external genital warts and some skin conditions, due to its cytotoxic effects. The use in anorectal disorders is primarily based on anecdotal reports and traditional texts rather than rigorous research. Moreover, Podophyllum resin can be highly toxic and cause severe adverse effects if misused, especially on mucosal surfaces.
