Nimbidin is a bitter principle extracted from the seeds of Azadirachta indica (Neem). Its use in the treatment of anal fistula and fissure is primarily rooted in traditional Ayurvedic and Unani medicine. Historically, Neem extracts, including nimbidin, have been valued for their anti-inflammatory, antimicrobial, and wound-healing properties. In Ayurvedic practice, Neem preparations are often applied topically or administered orally to promote healing of chronic wounds, including anorectal disorders such as fissures and fistulas. The rationale is that Neem’s bioactive compounds may help reduce local infection, inflammation, and pain, thus supporting tissue repair. However, the scientific evidence specifically supporting nimbidin for anal fistula or fissure is limited. Some in vitro and animal studies demonstrate that nimbidin possesses significant anti-inflammatory and antimicrobial activity, which could theoretically be beneficial for such conditions. Nonetheless, robust clinical trials or controlled human studies evaluating its efficacy in anal fistula or fissure are lacking. Most published literature references traditional use or extrapolates from general wound healing studies of Neem extracts, not nimbidin in isolation or in direct relation to these anorectal conditions. Therefore, while its use is justified by tradition and supported by plausible mechanisms, high-quality scientific validation is currently lacking.
