Vitamin E has been investigated as a treatment for Alzheimer's Disease (AD) due to its antioxidant properties. Oxidative stress is believed to contribute to neuronal damage in AD, and Vitamin E, as a fat-soluble antioxidant, was hypothesized to protect brain cells from this damage. Several clinical trials have evaluated the efficacy of Vitamin E supplementation in slowing cognitive decline or progression in AD patients.
The most notable clinical trial is the 1997 "Sano" study (NEJM), which found that high-dose Vitamin E (2000 IU/day) modestly delayed clinical progression in patients with moderate Alzheimer's Disease compared to placebo. However, the benefit was limited to delaying functional decline rather than improving cognition. Subsequent studies have produced mixed results, with some failing to show significant cognitive or functional benefits. A 2014 study (JAMA) reported a modest slowing of functional decline with high-dose Vitamin E in mild to moderate AD, but concerns remain about the clinical significance and safety of high doses, particularly increased risk of hemorrhagic stroke and all-cause mortality in some analyses.
Overall, while there is some scientific evidence suggesting a small benefit of Vitamin E in slowing the progression of functional decline in AD, the effect is modest, and high doses may carry risks. The use of Vitamin E for Alzheimer's is thus supported by limited scientific evidence, but it is not a standard or strongly recommended therapy in clinical guidelines.
