Vitamin E has been investigated as a treatment to slow the progression of Alzheimer's Disease (AD) due to its antioxidant properties. The rationale is based on the hypothesis that oxidative stress contributes to neuronal damage in AD, and vitamin E, as a fat-soluble antioxidant, might help protect neurons by neutralizing free radicals. Several clinical trials have explored this theory.
The most notable study is the 1997 trial by Sano et al. (NEJM), which found that high-dose vitamin E (2000 IU/day) modestly delayed the progression of AD in patients with moderate disease. Participants taking vitamin E showed slower functional decline compared to placebo over two years. More recent studies, such as the 2014 TEAM-AD trial, also suggested a small benefit in delaying functional decline, but results were mixed and not all studies confirmed these effects. Furthermore, concerns have been raised about high-dose vitamin E increasing the risk of mortality or other adverse effects, particularly in older adults.
Major guidelines (e.g., from the American Academy of Neurology) consider vitamin E as a possible adjunct for mild-to-moderate AD but caution against routine use due to limited efficacy and safety concerns. Overall, the scientific evidence is limited and inconsistent, and vitamin E is not considered a primary or strongly recommended therapy for Alzheimer's Disease.
