S-adenosylmethionine (SAMe) is a compound involved in methylation reactions in the body, including the synthesis of neurotransmitters and maintenance of cell membrane integrity. Interest in SAMe as a supportive agent for Alzheimer's disease (AD) arises from its role in methylation, as impaired methylation pathways and low SAMe levels have been observed in AD patients. Some preclinical and small clinical studies suggest that SAMe supplementation may have neuroprotective effects, potentially improving neurotransmitter synthesis and reducing neuroinflammation and oxidative stress.

However, the scientific evidence supporting the use of SAMe specifically for Alzheimer's disease is very limited and primarily preliminary. Most studies are either animal-based or involve small sample sizes, short durations, or focus on cognitive impairment more broadly rather than Alzheimer's disease per se. There are no large, high-quality randomized controlled trials that conclusively demonstrate efficacy in slowing cognitive decline or improving symptoms in AD patients. SAMe is better established for use in mood disorders such as depression, which sometimes co-occur with AD, but evidence for direct benefit in Alzheimer's disease is weak.

In summary, while there is some scientific rationale and a limited body of suggestive evidence, the overall support for SAMe in the treatment or support of Alzheimer's disease remains insufficient. More robust clinical trials are needed before clear recommendations can be made.
