N-acetyl-cysteine (NAC) has been investigated as a potential adjunctive treatment for Alzheimer's Disease (AD) due to its antioxidant properties and role as a precursor to glutathione, a key intracellular antioxidant. Oxidative stress is believed to play a significant role in the pathogenesis of AD, and decreased glutathione levels have been observed in patients with the disease. Several preclinical studies have demonstrated that NAC can reduce oxidative damage and amyloid pathology in animal models of AD. Small clinical studies in humans have explored the use of NAC for cognitive impairment and AD. For example, a pilot study published in 2004 (Adair et al., Neurology) examined the effects of NAC combined with a multinutrient supplement in patients with AD and found some modest improvements in certain cognitive measures, although the results were not definitive. A more recent randomized controlled trial (2018) tested NAC in patients with mild cognitive impairment, with some evidence of benefit but again, no strong or consistent effects.
Overall, while NAC is supported by a plausible biological rationale and some encouraging preliminary studies, there is currently insufficient large-scale, high-quality clinical trial evidence to establish its effectiveness for Alzheimer's Disease. Thus, the scientific evidence supporting NAC for AD is limited and best rated as low/modest (2 out of 5). More robust research is needed to determine its true clinical value in this context.
