L-carnitine, and more specifically its acetylated form acetyl-L-carnitine (ALC), has been studied as a potential treatment for Alzheimer’s Disease (AD) due to its role in energy metabolism and possible neuroprotective effects. The rationale for its use is based on the observation that L-carnitine facilitates the transport of fatty acids into mitochondria, supporting cellular energy production, which is often impaired in neurodegenerative diseases. Acetyl-L-carnitine can also cross the blood-brain barrier and may increase the availability of acetylcholine, a neurotransmitter important for memory and cognition. Several clinical trials and meta-analyses have evaluated acetyl-L-carnitine in mild cognitive impairment and Alzheimer’s Disease. Some studies have reported modest improvements in cognitive function, particularly in early or mild stages, suggesting a potential benefit. However, many of these studies have methodological limitations such as small sample sizes, short durations, or inconsistent outcome measures. Larger and more rigorous trials have produced mixed or negative results, with some systematic reviews (e.g., Cochrane) concluding that the evidence is insufficient to recommend its routine use in AD. In summary, there is some scientific basis and historical clinical research supporting the use of L-carnitine/acetyl-L-carnitine in Alzheimer’s Disease, but the quality and consistency of the evidence are limited. Thus, while it is not purely traditional, scientific validation remains weak (rating: 2/5), and it is not a standard treatment.
