Citicoline (also known as CDP-choline) is a compound involved in the synthesis of phosphatidylcholine, a key component of cell membranes in the brain. Its use in Alzheimer's Disease (AD) is primarily supported by scientific, rather than traditional, evidence. Several clinical studies and meta-analyses have investigated citicoline for cognitive impairment and dementia, including Alzheimer's Disease. Some studies suggest that citicoline may have neuroprotective effects, enhance neuronal membrane repair, and increase levels of neurotransmitters such as acetylcholine and dopamine. These properties theoretically could benefit patients with AD, a condition characterized by neuronal degeneration and neurotransmitter deficits.

However, the clinical evidence for citicoline’s effectiveness in AD remains limited and mixed. While some small trials and open-label studies have reported modest improvements in cognitive function and daily living activities, larger, well-controlled randomized trials have often failed to demonstrate statistically significant benefits compared to placebo. A 2021 Cochrane review and other systematic reviews conclude that, although citicoline is generally safe and well-tolerated, the current evidence does not robustly support its routine use for Alzheimer's Disease. Thus, citicoline is sometimes used as an adjunctive treatment in practice, but it is not considered a first-line or evidence-based therapy for AD. The overall evidence rates as weak to moderate (2/5), indicating a need for further high-quality research to clarify its role in Alzheimer's Disease management.
