Cistanche, particularly Cistanche deserticola, is a traditional Chinese medicinal herb historically used for its purported benefits on cognitive function and as a tonic for "kidney yang" deficiency. In recent years, several preclinical studies have investigated its potential for supporting or treating Alzheimer's Disease (AD). The active compounds in Cistanche, such as echinacoside and acteoside, have demonstrated antioxidant, anti-inflammatory, and neuroprotective effects in laboratory and animal models. Research suggests that these compounds may help reduce amyloid-beta plaque formation, improve synaptic plasticity, and attenuate neuronal apoptosis—processes relevant to AD pathology. Some animal studies have shown improvements in memory and learning in rodent models of AD following administration of Cistanche extracts. However, clinical evidence in humans remains extremely limited, with very few small-scale human trials and no large, high-quality randomized controlled trials to date. Thus, while there is a scientific rationale and encouraging preclinical data supporting the use of Cistanche for cognitive decline and AD, the overall level of evidence remains preliminary. The use is justified more by emerging scientific findings than by deep-rooted tradition specifically for Alzheimer's, and further clinical research is needed to determine its efficacy and safety in humans.
