2′-Fucosyllactose (2′-FL) is a human milk oligosaccharide (HMO) that has garnered scientific interest for its potential role in modulating the immune system, especially in infants. Some early studies and preclinical models suggest that 2′-FL may help shape the gut microbiota, support the maturation of the gut barrier, and modulate immune responses in ways that could theoretically reduce the risk of developing allergic diseases, including respiratory allergies. For example, 2′-FL has been shown to promote the growth of beneficial bacteria such as Bifidobacteria, which are linked to lower inflammation and a more balanced immune system. There is also evidence that 2′-FL can interact with immune cells, promoting the development of regulatory T cells and reducing Th2-mediated allergic responses in animal models.

However, direct clinical evidence supporting the use of 2′-FL specifically for the treatment or prevention of respiratory allergies in humans is still limited. Most of the available human studies regarding 2′-FL focus on its safety, tolerability, and its general benefits for infant gut and immune health. A few observational and intervention studies suggest a potential for reduced incidence of allergic symptoms in infants fed formula supplemented with 2′-FL, but these results are preliminary and not conclusive. As a result, the evidence for using 2′-FL for respiratory allergies is currently rated as low to moderate, with a need for more robust, large-scale clinical trials to confirm these effects.
