2′-Fucosyllactose (2′-FL) is a human milk oligosaccharide (HMO) that has garnered interest for its potential immunomodulatory effects, especially in early childhood. There is emerging scientific evidence suggesting a potential role for 2′-FL in supporting the development of the immune system and in reducing the risk of certain allergic conditions, including food allergies. Several preclinical and clinical studies have shown that 2′-FL may help shape the gut microbiota in favor of beneficial bacteria such as Bifidobacterium, which are linked to improved gut barrier function and reduced inflammation. These changes in the microbiome are thought to influence the maturation of the immune system and may lower the risk of developing allergies by promoting immune tolerance to dietary antigens.
Some animal studies and limited human trials have reported that supplementation with 2′-FL can modulate immune responses, including increasing regulatory T cells and reducing markers of allergic inflammation. For instance, infants fed formula supplemented with 2′-FL have shown immune profiles more similar to those of breastfed infants, who typically have lower rates of allergy. However, direct high-quality clinical evidence supporting the use of 2′-FL specifically for the prevention or treatment of food allergies in humans remains limited, and results are not yet conclusive. Most studies have been observational or have assessed surrogate markers rather than the incidence or severity of food allergy outcomes.
Overall, while there is promising scientific rationale and some supporting evidence, more rigorous and targeted clinical trials are needed to firmly establish the efficacy of 2′-FL for food allergy support or treatment. Thus, the current evidence rating is moderate but not definitive.
