Ornithine l-aspartic acid (LOLA) is primarily used in the treatment of hepatic encephalopathy—a complication of advanced liver disease often associated with chronic alcoholism. The rationale for its use is based on its ability to reduce elevated ammonia levels in the blood by enhancing the urea cycle and glutamine synthesis. While LOLA is not a direct treatment for alcoholism itself, it is scientifically validated for supporting liver function and managing hepatic encephalopathy in patients with alcohol-related liver disease. Several randomized controlled trials and meta-analyses have shown that LOLA can improve mental status and decrease ammonia levels in patients with hepatic encephalopathy, although effects on mortality and long-term outcomes remain uncertain. The European Association for the Study of the Liver (EASL) has included LOLA among therapeutic options for hepatic encephalopathy. However, there is no strong evidence that LOLA helps with alcohol dependence, withdrawal, or prevention of relapse. Its use in this context is limited to managing the liver complications of chronic alcohol use, not alcoholism per se. Therefore, while there is scientific support for LOLA's use in alcohol-related liver disease, it is not a treatment for alcoholism itself, and the overall evidence level for this specific indication rates as moderate (2/5).
