Nicotinamide riboside (NR) is a form of vitamin B3 and a precursor to nicotinamide adenine dinucleotide (NAD+), a crucial molecule involved in cellular metabolism, DNA repair, and mitochondrial function. As humans age, NAD+ levels naturally decline, which has been linked to age-associated physiological decline and diseases. The rationale for using NR as an anti-aging supplement is based on its ability to increase NAD+ levels in cells and tissues, potentially restoring or enhancing cellular health and resilience to age-related stressors. Preclinical studies in mice and other animal models have shown that NR supplementation can improve mitochondrial function, enhance physical performance, bolster neuroprotection, and even extend lifespan in some cases. These studies suggest that boosting NAD+ with NR may counteract certain aspects of aging at the cellular level. Human clinical trials, however, are limited and have mostly focused on the safety, bioavailability, and metabolic impact of NR supplementation. While several studies confirm that oral NR increases NAD+ levels in humans, evidence for concrete anti-aging effects (such as improved muscle function, cognitive performance, or reduction in age-related diseases) is still preliminary or lacking. No large-scale, long-term trials have yet demonstrated robust clinical anti-aging outcomes. Thus, while there is promising mechanistic and animal data, the current level of evidence in humans is modest, justifying a rating of 2 out of 5.
