MK-677 (also known as Ibutamoren) is a non-peptide growth hormone secretagogue that stimulates the secretion of growth hormone (GH) and insulin-like growth factor 1 (IGF-1). Its use as a potential agent to support or prevent aspects of aging is primarily based on its ability to increase GH and IGF-1 levels, which naturally decline with age and are associated with reduced muscle mass, bone density, and other markers of physiological aging. Several clinical studies have explored MK-677 in older adults. For example, a randomized, double-blind, placebo-controlled trial published in JAMA (2008) found that MK-677 administration in healthy older adults increased GH and IGF-1 concentrations to levels seen in younger adults and led to a modest increase in lean body mass after 12 months. However, improvements in functional outcomes, such as strength or mobility, were minimal and the long-term safety profile remains uncertain. Some studies have also reported side effects like increased appetite, transient insulin resistance, and edema. Overall, while there is scientific evidence that MK-677 can reverse some biomarkers of aging (e.g., low GH/IGF-1, loss of lean mass), there is limited evidence that this translates into meaningful clinical benefits for aging prevention or healthspan extension. Thus, the current body of evidence supports a cautious rating of 2 for its use in aging, indicating weak scientific support, warranting further research.
