Ibutamoren (MK-677) is a selective agonist of the ghrelin receptor and a potent stimulator of growth hormone (GH) secretion. Its potential application in aging (prevention) is based on its ability to increase circulating GH and insulin-like growth factor 1 (IGF-1), both of which decline with age and are implicated in some aspects of the aging process. Several clinical studies have investigated Ibutamoren in older adults. For example, a randomized controlled trial published in JAMA (2008) found that Ibutamoren administration in elderly subjects increased GH and IGF-1 to levels seen in young adults and resulted in modest improvements in fat-free mass, but did not show significant improvements in functional outcomes or quality of life. Other studies have reported increased muscle mass and improved sleep architecture, but no robust evidence exists showing that Ibutamoren actually slows or prevents aging, reduces morbidity, or extends lifespan in humans. Side effects such as increased appetite, edema, and insulin resistance have also been noted. In summary, while there is moderate scientific evidence that Ibutamoren can counteract some hormonal changes of aging, there is limited evidence supporting its use as an effective intervention for aging prevention or age-related diseases. Most experts consider the current evidence insufficient to recommend its use for this purpose outside of clinical research.
