Beta-hydroxy beta-methylbutyrate (HMB) is a metabolite of the amino acid leucine and has been studied for its potential to attenuate muscle loss (sarcopenia) associated with aging. The scientific justification for its use is based on several randomized controlled trials and meta-analyses suggesting that HMB supplementation can help preserve or increase lean body mass and muscle strength, particularly in older adults at risk of muscle wasting. The proposed mechanisms include reduced muscle protein breakdown and enhanced muscle protein synthesis.

However, while some studies demonstrate modest improvements in muscle mass and function, especially when combined with resistance exercise, the magnitude of these effects is generally small and not consistently observed across all studies. Large, long-term clinical trials specifically targeting functional outcomes, quality of life, or mortality in the elderly are lacking.

To date, major clinical guidelines do not universally endorse HMB supplementation for aging-related muscle loss due to inconsistent evidence and a lack of demonstrated long-term benefits. HMB is not a traditional remedy for aging; its use is entirely based on recent research. Thus, while there is some scientific support, the evidence is moderate at best and not definitive for aging prevention.
