Siberian ginseng (Eleutherococcus senticosus), also known as eleuthero, has a long history of use in traditional Chinese and Russian herbal medicine as an "adaptogen"—a substance believed to help the body resist physical, chemical, or biological stressors. The concept of "adrenal fatigue" is not recognized as a medical diagnosis in mainstream medicine, but it is commonly referenced in alternative health circles to describe a collection of nonspecific symptoms such as fatigue, body aches, and sleep disturbances, purportedly due to chronic stress overloading the adrenal glands.

Siberian ginseng has traditionally been used to increase energy, reduce fatigue, and improve resilience to stress, which aligns with the symptoms associated with so-called adrenal fatigue. Some studies suggest that Siberian ginseng may help modulate the body’s stress response by influencing the hypothalamic-pituitary-adrenal (HPA) axis and normalizing cortisol levels, though most research is preliminary, animal-based, or focused on general stress adaptation, not specifically "adrenal fatigue."

There is limited scientific evidence directly supporting the use of Siberian ginseng for adrenal fatigue itself, largely because "adrenal fatigue" lacks clear diagnostic criteria and is not widely studied. However, its traditional use for stress and fatigue, along with some supporting data for general adaptogenic properties, accounts for a moderate evidence rating, but robust, high-quality human trials are lacking.
