Vitamin B5, also known as pantothenic acid, is essential for the synthesis of coenzyme A, which plays a crucial role in fatty acid metabolism and energy production. "Pantethine" is a derivative of pantothenic acid and has been studied primarily for lipid-lowering effects, but there is some scientific rationale for its role in adrenal cortex support. The adrenal glands require coenzyme A for the biosynthesis of steroid hormones, including cortisol and other glucocorticoids. Early animal studies from the mid-20th century suggested pantothenic acid deficiency could result in decreased adrenal function and reduced production of adrenal hormones. Some small human studies and case reports have indicated that vitamin B5 supplementation may help with stress-related fatigue, which is sometimes attributed to suboptimal adrenal function. However, clinical evidence remains limited, and most modern studies focus on pantethine's effects on cholesterol rather than direct adrenal support. Therefore, while there is a plausible biochemical mechanism and some historical animal data, robust human clinical trials are lacking. Its use for adrenal cortex support is based more on basic science and tradition within integrative and naturopathic medicine than on strong clinical evidence.
