Triterpene saponins are a diverse group of naturally occurring compounds found in various plants such as licorice root (Glycyrrhiza glabra), ginseng (Panax ginseng), and others. Their use to support the adrenal cortex is primarily rooted in traditional herbal medicine practices, particularly in Traditional Chinese Medicine (TCM) and Ayurveda. In these systems, plants containing triterpene saponins—such as licorice and ginseng—are considered adaptogens, believed to help the body adapt to stress and support healthy adrenal function.

Despite their widespread use in herbal traditions, scientific evidence specifically linking triterpene saponins to direct support of the adrenal cortex is limited. Some studies on licorice root's active compound, glycyrrhizin (a triterpene saponin), suggest it may influence cortisol metabolism by inhibiting the enzyme 11β-hydroxysteroid dehydrogenase, which in turn could impact adrenal hormones. However, these effects are not uniformly beneficial and can lead to adverse effects such as hypertension and hypokalemia with prolonged use.

Beyond this, there is a lack of robust clinical trials directly demonstrating that triterpene saponins from any plant source provide significant support to the adrenal cortex in humans. Thus, while their use is justified by tradition, the evidence base remains weak and relies more on historical practices than on modern scientific validation.
