Caterpillar mushroom (Cordyceps sinensis) has a long history of use in traditional Chinese and Tibetan medicine, where it is believed to enhance vitality, stamina, and resilience to stress. These effects are associated with what modern herbalists would call "supporting the adrenal cortex," as the adrenal glands produce hormones critical for stress response, such as cortisol. However, the traditional texts and ethnobotanical records do not specifically mention the adrenal cortex, since this anatomical knowledge was not present historically. Instead, Cordyceps was used to combat fatigue, weakness, and to "replenish the kidney," which in traditional Chinese medicine encompasses adrenal function.

Modern scientific research has begun to investigate Cordyceps for its potential adaptogenic properties, including its effects on energy metabolism and stress. Some animal studies and limited human research suggest that Cordyceps extracts may help moderate stress responses and reduce fatigue, possibly by modulating the hypothalamic-pituitary-adrenal (HPA) axis. However, direct, robust evidence linking Cordyceps to specific adrenal cortex support in humans remains sparse. Most claims are still based on traditional use and preliminary research. As such, its use for supporting the adrenal cortex remains primarily justified by tradition, with some emerging but not yet conclusive scientific backing.
