Neem (Azadirachta indica) has a long history of use in traditional Indian (Ayurvedic) medicine for treating skin conditions, including acne. Its application is primarily based on its reputed antibacterial, anti-inflammatory, and wound-healing properties. Traditional uses include applying neem oil or paste directly to the skin or ingesting neem preparations. Ayurvedic texts describe neem as useful for “purifying the blood,” which is thought to contribute to clearer skin.

Scientific studies investigating neem's efficacy for acne are limited and generally of low quality. Some in vitro studies suggest neem extracts have antibacterial activity against Propionibacterium acnes, a bacterium implicated in acne development, and anti-inflammatory effects that could theoretically help reduce acne lesions. A few small clinical studies have included neem as a component in herbal formulations with reported improvements in acne symptoms, but these studies often lack rigorous controls and isolate neem's effects poorly.

Overall, while traditional use of neem for acne is well established, the scientific evidence is modest and indirect. More high-quality, controlled clinical trials are needed to determine its true effectiveness for acne treatment.
