Lactobacillus reuteri has garnered scientific interest as a probiotic that may help support skin health, including the management of acne. The rationale for its use is primarily based on emerging scientific evidence rather than longstanding traditional use. Several small clinical studies and preclinical investigations have explored the connection between the gut microbiome and skin health, leading to the hypothesis that probiotics such as L. reuteri may exert beneficial effects on acne through modulation of systemic inflammation, immune function, and skin barrier integrity.

Some studies have shown that L. reuteri supplementation can reduce systemic inflammatory markers and alter the composition of the skin microbiota. One small randomized controlled trial found that daily administration of L. reuteri reduced acne lesion counts and improved skin appearance compared to placebo. Animal model research and in vitro studies have also demonstrated anti-inflammatory and antimicrobial properties of this strain, suggesting it may inhibit the growth of Cutibacterium acnes (formerly Propionibacterium acnes), the bacteria implicated in acne pathogenesis.

However, the overall quality and quantity of evidence remain limited. Most studies are small, short-term, and sometimes funded by probiotic manufacturers. There is a need for larger, independent, and well-controlled clinical trials to firmly establish efficacy and optimal dosing strategies.
