Calcium pantethine is the stable, active form of vitamin B5 (pantothenic acid) and has been studied for its potential lipid-lowering effects, particularly regarding triglycerides. Several randomized, double-blind, placebo-controlled clinical trials have evaluated pantethine's role in lipid management. For example, a 2014 study published in Nutrition Research demonstrated that pantethine supplementation (600-900 mg/day) over 16 weeks resulted in significant reductions in triglyceride levels as well as total and LDL cholesterol in low-to-moderate cardiovascular risk subjects. Earlier studies from the 1980s and 1990s also reported triglyceride-lowering effects, though sample sizes tended to be small and methodologies varied.
The proposed mechanism involves pantethine’s role as a precursor to coenzyme A, which is crucial in fatty acid metabolism and may influence lipoprotein synthesis and breakdown. It is important to note that these effects are attributed specifically to pantethine, not to the more commonly available calcium pantothenate form of vitamin B5, which does not appear to have the same lipid-lowering properties.
While the evidence is promising and consistent across several small trials, larger, long-term studies are lacking, and the effect size is generally modest. Therefore, the evidence base is moderate (rated 3/5), suggesting that pantethine may be beneficial for triglyceride reduction, but it should not be considered a replacement for established therapies or lifestyle interventions.
