Dipeptidyl peptidase IV (DPPIV) is an enzyme with a well-characterized role in the digestion of proline-containing peptides, particularly those found in gluten and casein, within the small intestine. Scientific research supports that DPPIV is naturally present on the brush border of the small intestinal mucosa and contributes to the final steps of protein digestion by cleaving dipeptides from the N-terminus of polypeptides. This function is especially relevant in the hydrolysis of immunogenic gluten peptides, which are otherwise resistant to breakdown by other proteases. Some dietary supplements include DPPIV to support individuals with gluten or casein sensitivities, with the hypothesis that supplemental DPPIV may enhance the body’s ability to degrade these problematic peptides and reduce digestive discomfort.

However, while in vitro studies and limited clinical investigations suggest that DPPIV can degrade certain gluten peptides, the clinical evidence for its effectiveness in humans—especially in those with celiac disease—is lacking or inconclusive. Regulatory and medical organizations do not endorse DPPIV supplementation as a treatment for celiac disease or other serious digestive disorders. The evidence is somewhat stronger for its general role in protein digestion, but less robust for therapeutic supplementation. Therefore, the use of DPPIV to support small intestine function is scientifically plausible and partially validated, but the quality and quantity of clinical evidence warrant a moderate rating.
