Serratiopeptidase is a proteolytic enzyme originally isolated from bacteria in the intestines of silkworms and has been used in some countries as an anti-inflammatory and mucolytic agent. Its use for supporting the sinuses primarily hinges on its ability to reduce inflammation and break down mucus, which could theoretically help relieve symptoms associated with sinusitis or other upper respiratory conditions. Several small-scale clinical trials and observational studies, mostly conducted in India and Japan, have suggested that serratiopeptidase may help decrease nasal discharge, facial pain, and congestion by reducing inflammation and thinning mucus. For example, a few studies have reported improvements in symptoms of chronic or acute sinusitis when serratiopeptidase was used as an adjunct to antibiotics or other standard therapies.
However, the overall quality of evidence is low. Most studies are limited by small sample sizes, lack of blinding, or inadequate controls. Major scientific guidelines in the US and Europe do not list serratiopeptidase as a recommended treatment for sinus problems, and there is a lack of large, well-designed randomized controlled trials. While there is a plausible mechanism and some early clinical data, the evidence is not robust. Therefore, serratiopeptidase's use for sinus support is scientifically plausible but not strongly validated.
