Pelargonium, particularly the species Pelargonium sidoides, has a well-documented history of use in traditional South African medicine for respiratory ailments, including sinus and upper respiratory tract infections. In recent years, scientific studies have investigated its efficacy, most commonly in the form of the standardized extract EPs 7630. Several randomized controlled trials and meta-analyses have found that Pelargonium extracts can reduce the severity and duration of acute rhinosinusitis and the common cold, supporting its use for sinus health. The mechanism is thought to involve both mild antimicrobial effects and immune modulation, as well as promoting mucociliary clearance. While most studies focus on acute, uncomplicated sinus issues, the evidence base is considerably stronger than for many other herbal remedies. However, the quality of evidence varies, with some studies showing modest benefits and methodological limitations. The use of Pelargonium for chronic sinus issues or in populations with significant comorbidities is less well supported. Overall, there is moderate scientific validation for short-term use of Pelargonium extracts in supporting sinus and upper respiratory tract health.
