Calamus (Acorus calamus), commonly known as sweet flag, has been traditionally used in Ayurvedic and other traditional systems of medicine for a variety of ailments, including those related to the respiratory tract and sinuses. Historical texts describe its use as a remedy for sinus congestion, coughs, and colds, often administered in powdered form, as an inhalant, or as a part of herbal formulations. The rationale for its use in supporting the sinuses body system is rooted in its aromatic properties and traditional beliefs that it helps to clear congestion and act as an expectorant.

However, while these traditional uses are well documented, there is very limited modern scientific research specifically validating calamus for sinus support. Most available studies on calamus focus on its potential neuroprotective, anti-inflammatory, and antimicrobial properties, but direct clinical evidence for its effectiveness in treating sinus issues is lacking. Some laboratory studies suggest that calamus has antimicrobial effects, which could theoretically help with infections that affect the sinuses, but these findings have not been robustly translated into clinical practice. In summary, the use of calamus for sinus support is primarily justified by traditional practices rather than scientific evidence, and the strength of evidence is weak.
