Estrogen is scientifically validated as a treatment for low sex drive (hypoactive sexual desire) in certain populations, particularly in postmenopausal women. After menopause, estrogen levels drop significantly, often leading to vaginal dryness, discomfort during intercourse, and a decrease in libido. Estrogen therapy—administered as systemic hormone replacement therapy or localized vaginal estrogen—can alleviate these symptoms by restoring vaginal tissue health and increasing lubrication, which can indirectly and sometimes directly improve sexual desire and function.

Randomized controlled trials and systematic reviews have shown that estrogen therapy, especially when combined with other hormones like testosterone, may improve sexual function scores in postmenopausal women. However, estrogen alone is less consistently associated with increased libido compared to testosterone therapy.

For premenopausal women or men, estrogen is not typically used to treat low sex drive, and may even reduce libido in men. The use of estrogen for this indication is therefore limited to specific cases where hormonal deficiency is clear.
